Children with allergies, asthma, and/or seizures(requiring action on our part)

All children enrolled will need these 3 forms.

  Older versions or different forms will not be accepted.

REGISTRATION

  A $100 registration fee is required to save your child's spot.

 Payment can be made at the bottom of this screen now or when you are finished filling out the form.  You will not be automatically directed to pay.

Classes on a waitlist are indicated as such on the registration page.  Please do not pay if you are on the waitlist.  If a spot opens up, we will collect the registration fee at that time.
